Prepare a handover sheet for pet care
Use these columns to keep information about your own project. The sheet is blank; you can print it and fill it in.
- Prepare the animal’s own identification, known habits and available supplies. Describe what the carer should find or ask about rather than copying another pet’s routine.
- Add useful contacts and the location of necessary documents. For prescribed care, pass on the veterinarian’s instructions instead of an improvised summary. Check that the carer understands the task.
- Review the sheet together before the handover and clarify uncertainties. Keep dated questions and changes. Share it only with the people responsible for care.

Notes
The sheet replaces neither a responsible carer nor veterinary advice. The UK source supports preparation without extending its legal framework elsewhere.
