Prepare an identification record for your pet

Gather your animal’s references in a private sheet linking back to original documents. The aim is to prepare a conversation with a service or veterinarian while distinguishing registered identity, contact details and health records.

The method

  1. Find documents you hold and record the name used, identification reference and issuing organisation. Compare references when several documents concern the same animal.
  2. If your animal is covered by I-CAD in France, consult its instructions for retrieving the digital card from your own keeper account. Keep the document and date without sharing account access.
  3. Record missing or conflicting details to confirm. Keep personal contacts and complete references in the private version; prepare a limited version when asking for information.

Fictional worked example

In a fictional example, the record names “Tao” while an older document uses another name. The sheet retains both names, document origins and the reference to compare. It does not merge records based solely on a similar name or photograph.

The record to keep

Keep the sheet, source documents and questions. Use the relevant service’s procedure for changes to an identification register.

What this method can establish

The sheet replaces neither official procedures nor the animal’s medical record. A reference inconsistency needs confirmation from an appropriate contact.
